It usually starts with a vague sense that something’s off. Fatigue that’s not your normal fatigue. Joint stiffness. Brain fog. Maybe a rash or some odd symptoms that come and go. You finally push for labs — and then comes the result that gets your attention: a positive ANA.

It’s a red flag. One that could point toward autoimmune activity — lupus, Sjögren’s, mixed connective tissue disease, or others. Your PCP refers you to rheumatology. You wait weeks, sometimes months, for the appointment.

And then? The specialist reviews your labs, maybe examines you briefly, and says something like:

“Your ANA is positive, but everything else looks fine. Let’s just monitor things. Come back if new symptoms develop.”

In other words:
“You seem fine. Let’s wait until you’re not.”

But here’s the thing:

A positive ANA — even without a clear autoimmune diagnosis — is not nothing. It’s your immune system raising its hand. It’s a signal that something is off-balance. The body doesn’t just start making autoantibodies for fun.

The real question shouldn’t be “Which autoimmune disease will you develop?”
It should be:
“What’s driving this immune dysregulation in the first place?”

Because the conventional “wait and watch” approach can feel like gaslighting — a dismissal of symptoms that are real, uncomfortable, and often disruptive to daily life. It’s passive. It assumes disease is inevitable, and that there’s nothing worth doing until you’re sicker.
